1. Japan Regulatory Bundle Application: An application must be completed and signed by the authorized representative.
2. Proof of Business Identity: Corporate registration document.
3. Proof of Business Address (Must be within the locality or region covered by the phone numbers prefix): Corporate registration documents. If your Corporate registration document does not state your Business Address, please provide one of the following to prove the local Business Address:

   • Certificate of tax payment of the local corporate entity.

   • Utility bill of the local office address

4. Proof of Business Purpose: Corporate registration documents showing declaration of purpose.
5. Proof that the representative is authorized to act on behalf of the business: Power of attorney, OR Corporate registration showing the name of the authorized representative, OR A Letter of the company letterhead under seal showing the person is duly authorized to execute the contract with us.
6. Proof of Identity: A valid identification is required to verify the authorized representative’s name, photo, date of birth, and current address.

• My number Card

• “Zairyu” residency card (non-Japanese national)

• Certificate of special parliament resident (non-Japanese national)

7. Proof of Address: Provide one of the following documents showing the current address of the Authorized Representative:

   Certificate of tax payment

** Supporting documents must have been issued within the last 6 months and be valid at the time of upload.
